Hospital in Salem, Oregon, burial I.O.O.F cemetery Dec. 23rd by Terwillier, informant: Hosp. Records. Terwilliger Records confirm the death certificate information.
OBITUARY: 
At a local hospital, December 21, Albert Lewis, Jr., formerly a resident of this city, age 17 years. Survived by his father, two sisters, Mrs. C.A. Leach, Raymond, Wash., Mrs. Harvey Behnke, Portland, and three brothers, Leonard and Ray of Raymond, Wash., and Ben of Salem. Funeral will be form the Terwilliger home Sunday at 2 p.m.
